Yahoo! News Full Coverage - Science - Senators Rip NASA Over Shuttle Columbia Probe I don't think its engineer's fault its manament who is in charge of safety. Alot of the early media mentioned engineers talking about it in the past(the foam and other safety issues). A recent article I read suggested that the safety around NASA hasn't changed much since Challenger.
